Sign Up Information
Sign-Ups will be the last weekend of January and the first weekend of February at the Miami Valley Center Mall right outside of the Sears Store. You may download a sample form below, but please understand that you will be required to complete our form at the Piqua Mall. We are sorry for the inconvenience, but we must have you complete the triplicate form.
You may download a sample sign-up form, but you will be required to fill out the form during Sign-Ups at the mall.

T-Ball - ages 4-6. Child MUST be 4 years old as of January 1.
G-Ball - ages 7-8.
Midget- ages 9-10.
